The SNAP food benefits (formerly known as Food Stamps) helps people with low incomes and resources buy the food they need for good health. Medicaid Programs provide medical benefits for eligible low income adults, children, and families. CHIP offers a health insurance program for children. Intake Requirements Documents required for application are varied and depend upon the Benefit program. They may include, but are not limited to: Social Security number; proof of citizenship or immigration status; pay stubs or bank statements to show proof of income; award letters or pay stubs for other benefits such as Social Security, pension, veterans benefits, unemployment, or workers' compensation; child support payment or receipt documentation; tax statements; asset documentation, such as mortgage note, or royalty statements; insurance cost and policy documentation.
